1. If you require a Social Welfare Cert – Illness & Injury Benefit Application Form and you have a verified or basic MyGovID account you can apply online on MyWelfare.
2. If you cannot apply online , you can download the IB1 form below. This needs to be done once only to start your Illness & Injury Benefit claim. Please then fill out the form below which is required for a Doctor to submit a Med1 e-cert to the Department of Social Protection.
